  • Original F137 (with request letter from CDL)
  • Original Certificate of Good Moral Character issued in the current year
  • Two (2) identical copies of recent 2x2 studio photo (white background)
  • Clear photocopy of PSA Birth Certificate
  • For married female applicants, clear photocopy of Marriage Certificate
  • Clear photocopy of Grade 12 Senior High School Diploma

Transferee / Second Courser Requirements

For evaluation of subjects that can be credited from your previous school, submit any one of the following:

  • Scholastic Records
  • Certificate of Grades
  • Class Cards
  • Transcript of Records
  • Online Copy of Grades

Other Requirements:

  • Honorable Dismissal / Transfer Credentials
  • Transcript of Records (TOR) copy for CDL
  • Two (2) identical copies of recent 2x2 studio photo (white background)
  • Clear photocopy of PSA Birth Certificate
  • For married female applicants, clear photocopy of Marriage Certificate
